How do I talk to my doctor?

I’ve been struggling with my life for the past 7-8 years but I finally made an appointment with a doctor. I feel really scared though because I don’t ever go to the doctor for anything, and I havent seen this doctor for so many years that I’m really scared that they’ll judge me or won’t believe anything I say.
